COMMUNITY & MENTAL HEALTH PROFESSIONALS

Mental Health Community Provider Database

The Campus Social Worker office maintains a database of community providers (psychotherapists, psychiatrists and community clinics) who can provide services to our students. Therapists on our list must meet minimal qualifications, including but not limited to having active California licensure (i.e. clinical psychology, MFT or LCSW) and proof of malpractice insurance. If you are interested in applying to be part of our referral database, please create an account and complete an application on the following website: http://www.whcs.uci.edu/community-provider-database. Please allow approximately 30 minutes to complete the following form in full as there is no option to save your work along the way. Submission of your information does not guarantee inclusion on our database. All submissions must be reviewed and approved by the Campus Social Worker office.
