You can apply to be part of our referral database by creating an account and completing an application. Please allow approximately 30 minutes to complete the form in full; there is no option to save in-progress.

Therapists on our list must meet minimal qualifications, including having active California licensure (i.e., clinical psychology, MFT or LCSW) and proof of malpractice insurance. Submission of your information does not guarantee inclusion on our database. Submissions must be reviewed and approved by the Campus Social Worker.
